Comments: Construction of the new central kitchen site in westport site is ongoing. The building structure and roof are now intact. It is scheduled for completion in May 2018. Processes that are expected to change include a quick chill process where meals get delivered to sites cold and microwaveable containers for rapid reheating. “Oliver system packaging” is the process which is going to be used. The existing walkin coolers have been removed from service and deconstructed. A walkin cooler was purchased and installed without consulting DOH, located on the east side exit to the building. A wooden enclosure was constructed that is not finished and sealed from the outdoors such that flies and rodents can enter during hours of operation. Additionally, it reportedly leaks when it rains such that it can drip on carts. Site forms and weekly "last meal temp" forms were reviewed for each site and they were found to be acceptable. The importance of food temperatures and time were discussed, along with corrective actions for food received out fo temperature. Currently, schroon, Newcomb, Minerva and St. Armand are preparing full meals at their sites. Minerva and St. Armand have the longest travel times for meals delivered to sites which can exceed 2 hours. Minerva site is in the process of building a new site. Per interview, the assigned DOH inspector was notified and is working towards approval. The Monthly, menu was reviewed and cooking and cooling procedures for leftovers was reviewed. Frozen meals get delivered to homes where reheating instructions (350-400 °F uncovered for 10-30 min), do not specify a temperature. With the instructions provided it is unlikely that temperatures would reach 165 °F. You should amend reheating instructions to include reheat temperature to 165 °F in 2 hours. Food design consultants, site visit/equipment assessment and all aspects of the renovation and operation during work must be done in compliance with 14-1 of the public health code and approval from DOH. During the review phase, septic plans or a septic evaluation done by a NYS licensed professional engineer must be submitted to DOH for review. Comments: Roast beef, vegetables and potatoes were observed being plated for sites and home delivered meals. All temperatures were at or above 180 deg. F. Four air filters are situated throughout the facility and are maintained by the site. Filters are changed every couple of weeks. The fan grates to the top of the filters were collecting dust and may need more frequent cleaning. The air filters were reportedly there to remediate air quality and were maintained by an outside company. No evidence of significant mold growth was observed and food or nonfood contact surfaces. Cooling logs were reviewed and some cooling temperatures were not carried out to 45 degrees F or below. As cooling slows as you approach 45 degrees F it is important to track cooling until it reaches that temperature. Per interview and observation, cooling techniques were adequate to accommodate proper cooling. Site renovation plans are in development. Older plans that have since been updated were added to the file. You must complete a notice of intent form and submit official plans that must be approved by our office prior to making any purchases or improvements. Home delivered meals and site meal logs were reviewed and found to be acceptable. The dishwasher was working and thermometers were accurate. Many of the structural violations are repeat violations that will need a time table of compliance. Newcomb, schroon lake and minerva are the only sites that prepare meals.
