Comments: 5C - About 1 pound of fried chicken nuggets were found sitting on counter at room temperature, temped chicken at 84 degrees F using inspector's digital thermometer. Manager stated that chicken had been left out for a few hours. Manager voluntarily discarded chicken. CORRECTED. 14C - Insecticide found on premises, under front counter. Manager stated that it was just bought, and was used only for outdoors. Manager removed from permises. CORRECTED. 8A - Foods throughout kitchen were found uncovered. Plastic grocery bagswere used to cover some foods in upright freezer, a non food-grade material. 8A - Boxes of pop and boxes of vegetables were found stored directly on the floor (back room and in walk-in cooler). Need to be six inches off the floor. 10A - Flour found stored in original bag within a garbage can with a broken lid (cracked plastic). Manager stated that he would discard the lid and buy a new lid. Rice was found stored directly in a garbage bin, with grime buildup within can and around edges. Discussed proper food storage, explained that original bag may be used in place of garbage bin. 11D - Unclean surfaces were found in upright freezer, ceiling and walls of walk-in cooler, and handle of walk-in cooler. Food debris was found on lids of food storage bins (flour, salt, etc.) 10B - cardboard found lining shelf in kitchen, near 3 bay sink. REPEAT. 12C - Vegetable sink is inaccessible, being used for storage. 11C - No chlorine present in sanitizing bucket. Sanitizer was added at 100 ppm. CORRECTED. 14A - Flies present in kitchen.

I directed him to have under running water instead of standing water for thawing purposes. Corrected. 10B--Bottom shelf of prep table has cardboard lining it and the floor of the walk-in cooler is lined with cardboard. These surfaces are not smooth and easily cleanable. 11A--3 bay sink set up is found to be--Rinse, wash, sanitize, air dry. Technique must be Wash, Rinse, Sanitize, Air dry. 16--CPR kit is present under the counter, but there is no posting up in the dining area.
