Comments: Observed functional thermocouple. Observed hamburger patties and sausage at 148F in hot holding on prep line. Observed egg patties at 145F. Observed sliced tomatoes at 41F and sliced Swiss cheese at 42F on cold holding unit on prep line. Observed raw hamburger patties at 43F in refrigeration drawer under grill. Observed commercially prepared bacon onion sauce at 38F in walk-in cooler. Observed accurate thermometers in the walk-in cooler and the walk-in freezer. Observed proper glove use while preparing ready to eat food. Observed ice scoop in separate container on top of ice machine. Observed clean restroom facilities, appropriate hand-washing signage and adequate supplies (hot water, soap and hand dryer). Per GM, pest control operator conducts monthly inspections.

Comments: Facility has functioning thermocouple, which staff was using to evaluate the temperature of grilled chicken breast at 182F. Observed sausage in hot holding unit to have a temperature of 150F. Temperature of chicken tenders in walk-in cooler were 41F. Sanitizer (quaternary ammonia) in 3-bay sink was 200ppm, per facility's test strips. Observed staff wash, rinse, and sanitize hot holding trays during time of inspection.

Comments: Following violations were corrected at time of re-inspection: 5E chicken tender cooler was running at 38 F, 10B Outside surfaces of ice storage bin at drive through service area were cleaned, 11B Wiping cloths stored in sanitizing solution of quaternary ammonia were at a level of 200ppm,12C Waste line for soda mix machine terminates at floor drain, 15C-floor tiles at "crew room" and around soda mix machine were repaired. Discussed continued maintenance of floor with Bill, McDonald's maintenance crew.

Comments: Time table of correction set for all violations is 11/7/17. Sanitarians observed proper glove use to prevent barehand contact. Sliced tomatoes in service cooler had a temperature of 41 F. Large hamburger patty in hot holding service unit had a temperature of 150F, small patty approximately 140F. Advised manager to ensure hot-holding is adequate to maintain 140F+. Discussed ill worker policy. Reviewed waiver policies with manager and food worker. Discard times are written on dry erase board on prep counter counter.

Comments: Interview with manager indicated that the only wavier facility is using is for cheese. Facility is adequately cold holding sliced tomatoes, raw shell eggs, liquid eggs and adequately hot holding grilled chicken. Facility had fire in fryer on 8/3/15. Fire was contained and extinguished. Facility is no longer using fryer and is to be replaced. No contamination to food observed.
