AN INSPECTOR ALSO OVSERVED THE FACILITY OPERATING WITHOUT A VALID PERMIT ON JULY 7, 2017 AND LEFT A WRITTEN REPORT FOR THE OWNER WITH THE BARTENDER THE COMPLEATED RENEWAL APPLICATION, PROOF OF WORKERS COMPENSATION AND DISABILITY INSURANCE, NOTARIZED CERTIFICATE OR AUTHORITY, FEE OF $420 AND LATE FEE OF $80 MUST BE HAND DELIVERED TO THE MOUNT KISCO OFFICE LOCATED AT 25 MOORE AVENUE, MT KISCO NY 10549. THE FACILITY CANNOT OPEN FOR BUSINESS UNTIL THE APPLICATION IS REVIEWED AND AN INSPECTOR REMOVES THE NOTICE OF CLOSURE SIGN.
