Comments: Health Dept notified of fire at Taco Bell this morning approx 0800. Made field visit to premises at 0835. Facility was already closed and in the process of starting to clean up after the fire (Fryer caught fire, manager pulled alarm for Ansel unit and fire was extinguished. Fire Dept was contacted as a precaution. Exposed food on counters was disposed of. All utensils and containers in the area were rewashed. All surface area's were cleaned and sanitized. Told mgmt to contact Health Dept when ready for an inspection to re-open. At 3pm - re-inspected Kitchen and fryer. Everything was satisfactory. Allowed to re-open.

Comments: Received complaint on Nov 28, 2016 via telephone call of ice cubes in packaged meal received from drive up window. Complaintant was at Taco Bell on Nov. 27, 2016 at approx 01:30 am. Observed a female employee (Shelly with purple hair) and a male employee the throwing ice around the prep area when the Complaintant drove up to the drive-up window. When Complaintant got home... he noticed the box of food was cold, and soaking wet... with an ice cube at the bottom of the box. Complaintant alleges he went to Taco Bell the next day and found himself talking to the same individual (Manager) who was throwing ice the night before. He filled out a complaint report and was reimbursed $ for his food. Made site visit on Nov 30, 2016 and spoke with Manager -Shelby (with purple hair). She stated she was there that night and the male employee was throwing ice.I cautioned her in that ice is considered a food and if bare hands were used to throw the ice it could of contaminated other surfaces and food products. Education provided.
