Comments: Pesticide chart shows regular service by Bug Runner. The chart shows treatment for mice. Chart shows no recent service for roaches. Last treatment for roaches was application of Talstar spray in May. Treatment since that date has only been for roaches and only “GB” (glueboards?) placed. Only three glueboards observed at this time. Insects (spiders, ants) and one mouse dropping observed on the boards. Operator was instructed to thoroughly clean the basement. The removal of the mouse droppings is necessary in this food preparation and storage area to determine if rodent activity is still present. The operators Food Safety Certificate has expired. The next course is Oct. 9&10 and the operator has been scheduled by mail. This report was completed in the office on 9/28/18. A paper copy was left with the operator upon completion of the inspection on 9/27/18.
