Comments: The purpose of this inspection was to confirm that the pizza prep cutting board that the operator stated was replaced with a new cutting board via email with picture was in current use. Upon inspection of the pizza prep cutting board, it was noted that the previous cutting board that was deeply gouged and stained with food debris was in use. An inspection of the facility was conducted and the new cutting board was not stored anywhere on premises and was confirmed by the manager on duty. Photographs of the cutting board in use have been retained by the Department. After the owner was notified of the re-inspection for the cutting board, the manager was notified it was being stored in the office. The above operator has not reported and acted in good faith to the Department. Due to this issue the violation will be reviewed by the Food Service Program Lead and District Director for possible Enforcement Action.

Comments: The purpose of this pre-operational inspection is due to the facility relocating their current cook line to the back wall area. All items to be moved including hood, fryers, oven and other items. The operator has been informed that all hand wash sinks are to be conveniently located to all work area's. The owner has been directed to complete the following items: - Contact the Department before any further work to the kitchen area is completed. - Provide an updated C of O or C of C from the CEO - Complete the installation of the cook line and all other required equipment. - Remove all construction debris then clean and sanitize all surfaces. - All surfaces are to be smooth, cleanable, and durable. - All construction must be completed in accordance with Subpart 14-1 of the State Sanitary Code. - Contact the Department to schedule the final pre-operational inspection when all requirements have been completed. - When work is started in the kitchen area all food service is to be suspended until a satisfactory pre-operational inspection is completed by this Department.
