A re-inspection will be conducted after this date to determine compliance. Regarding the ceramic floor tiles which are in disrepair or missing in kitchen cook line area, a Time Table of Compliance Date of December 14, 2015 has been established to correct the floor tiles which are missing or in disrepair. Per Manager, facility is scheduled to change to a new operator in near future. Contact Health Dept. to obtain food service permit application paperwork and worker's comp. and disability forms which must be resubmitted prior to change of operator.
