Discussed not filling inserts in top of cold holding make units above indent line of inserts. Identified which foods in top of sandwich make unit were time and temperature control for safety/potentially hazardous food (TCS/PHF)- meats, cheese and sliced tomatoes. Observed chopped chicken in top of cold holding pizza make unit at temperature of 30F. Reviewed hand washing procedures and glove/utensil use. Reviewed waiver policy with staff, which is being followed.

Comments: Reviewed cooling procedures for sausage links with staff. Discussed cooling without lid to meet cooling schedule of 120-70 in 2 hours and 70-45dF in an additional 4 hours. Reviewed calibration procedures for probe thermometer- facility uses ice water bath. Reviewed cold holding temperature of 45dF or less. Recommended turning down sandwich make unit- temperature of food (ham slices and olives) was 46dF per sanitarian's thermocouple. Observed staff following requirements of waiver. Discussed hand washing between glove changes or any time of contamination.

Comments: Observed accurate probe thermometers throughout facility. Discussed hand washing and glove use. Diced tomatoes in make unit temped at 39dF. Cheese and sausage in make unit temped at 42dF. Facility had 6 cartons of Horizon Milk in cooler with best by date of November 2016. Milk cartons were sealed at temperature of 34dF and did not appear or smell to be spoiled. Staff voluntarily discarded 6 cartons.

Comments: Conducted a follow-up re-inspection to check on sewage waste material which was evident near clean-out cap in parking lot on 3-24-16. Dried sewage waste has been cleaned-up near clean-out cap near parking lot. Manager was instructed to keep this area clean whenever service work is done to prevent complaints from customers. Corrected.

Comments: LIght shields inside glass-doored refrigerator at front counter- end caps have been installed on shields. A Time Table of Compliance Date of May 30, 2016 has been established to correct the above-mentioned violation (Blue Item 12C). A re-inspection will be conducted after this date to determine compliance.

The following violations were corrected from the previous inspection; 8a-floor stripper not stored with drink bottles. 11d-pizza prep storage unit (area between cutting board and cover), cleaned. 15a-floor below deep fryers has been cleaned.

A Time Table of Compliance Date of February 10, 2016 has been established to correct violations listed above. A re-inspection will be conducted after this date to determine compliance.
