Comments: Two outstanding Administrative Tribunal items (signed by facility owner) are still outstanding: 1. agreed to have been completed by November 30, 2017: Repair the holes in the sheet rock wall(s) leading to the basement storage areas, and the wall at the dishwashing station, leaving these surfaces smooth and easily cleanable. 2. agreed to have been completed by April 30, 2018: Immediately cease food service in the basement. AND Remove the Alto-Sham unit(s) from the basement. Notes: The chemical sanitizing dishwasher located in the kitchen is working in accordance with the manufacturer specifications. The inspector noticed flames coming out of the top (rear) section of the broiler unit, within a few inches of the metal-coated gas line. The inspector discussed this potentially dangerous/deadly situation with both onsite staff.

Comments: A reduced oxygen packaging machine (Food Saver brand, residential grade) was observed plugged in on a shelf in the hallway between the kitchen and the waitstaff area. No reduced oxygen packaged food were observed onsite. The inspector and the onsite staff discussed the machine/process and the inspector informed them that the use of these machines without proper prior approval is against NYS Sanitary Code and a waiver is required from our office prior to any further use of this device. This device needs to be removed from the premises or locked in the cage in the basement until the facility obtains the above mentioned waiver from our office.
