Informed Andy that he will need to submit a new Permit Application, as well as a Notice of Intent for any renovations planned. He must also submit specifications of any new equipment that will be installed, as well as a sketch of the new floor plan. PERMIT WILL NOT BE ISSUED UNTIL A PRE-OPERATIONAL INSPECTION IS PERFORMED. At pre-operational inspection, it must be determined by NYSDOH that ALL outstanding and reoccurring violations have been satisfactorily corrected.
