Comments: Conducted a re-inspection due to previous violations listed from pre-operational inspection conducted on May 31, 2018. Regarding Blue Item # 12 C- Sink fixture was repaired at bar sink, regarding Blue Item # 15 A- Section of wall behind kitchen refrigerator has been repaired- Corrected. Will conduct a HACCP training with Operator later in the year.

Comments: Violations from previous inspection have been corrected. Operator cleaned all areas that were observed to contain rodent feces at time of previous visit. Operator also sealed (with spray foam and steel wool) holes that were observed in basement wall. No identifiable entry points observed inside building, including basement, first and second floors. Recommended operator request at least initial visit from pest control operator as treatment application and monitoring traps would aid in locating point of entry, if any remain. Advised operator that pest/rodenticides require licensure from NYSDEC when used in commercial buildings.

Comments: Reinspection conducted to verify correction of violations observed during inspection of 12/3/2015. All violations, including the following, were corrected: 11C-food containers and utensils which had contained buildup of grease have been cleaned, interiors of refrigerators have been cleaned; 11D and 15A-floors, shelving and equipment in kitchen have been cleaned of grease, dust.
