The facilitys permit is valid for bar service only. An air fryer is present at the bar. Frozen foods stored in a freezer in the rear are cooked in the air fryer and served on disposable plates. All utensils associated with the air fryer are washed, rinsed and sanitized in the 3-bay sink. A kitchen is present on-site but it does not currently comply with Subpart 14-1 requirements. If the facility wishes to obtain a food service permit, improvements outlined in the April 14, 2017 inspection report. During the inspection, the new smoking room was observed. Please submit smoking waiver application for this new room. As required for the previous smoking room, the new smoking room must have self-closing doors that isolate it from the rest of the building, an acceptable exhaust fan (rated for the square footage of the room) and a in-room smoke filtration unit (rated for the size of the room). Please submit information on proposed exhaust fan and smoke filtration unit to DOH for review and approval along with square footage of the room. |

Item 1H (Critical): Food from unapproved source, spoiled, adulterated on premises. Item 12C: Plumbing and sinks not properly sized, installed, maintained; equipment and floors not properly drained Item 8A: Food not protected during storage, preparation, display, transportation and service, from potential sources of contamination (e.g., food uncovered, mislabeled, stored on floor, missing or inadequate sneeze guards, food containers double stacked) Comments: The facility is considering applying for a food service permit. A review of the current kitchen facilities was conducted in conjunction of the kitchen. The following items were noted during the review of the kitchen: 1) Part 14-1 of the State Sanitary Code has a sink requirement. Currently the kitchen has a 2 - two bay sinks. One of those sinks is used as a mop sink. A dishwasher is not present. If the facility plans to expand operations to include food service, please submit plans for the proposed kitchen. An email outlining items to be considered on the plan was sent to the operator on 4/10/17 and a copy was provided on 4/14/17. Possible sink configurations were discussed during the on-site visit. Sinks not in use for food storage, preparation or processing or not in use for utensil or dishwashing may be used for hand washing. An indirect drain (air gap) will be required at the sink being used for food preparation. 2) The floor in the dry good storage room located behind the kitchen is constructed on unfinished plywood. The flooring is not considered smooth and easily cleanable. 3) Some deep cuts were noted on the butcher block counter top. 4) Please coordinate with Code Enforcement Officer regarding ventilation and grease trap requirements. Also, please contact codes regarding opening the basement space for public use. 5) If the bar in the basement is going to be used, a three bay sink and hand wash sink is needed. 6) Black mold was observed on some ceiling tiles in the basement. The mold is likely related to a groundwater intrusion problems that was noted during the visit. The facility has hired a contractor to address this problem. When replaced, the ceiling tiles above the bar (currently acoustic tiles) should be replaced with commercial restaurant grade tiles that are smooth and easily cleanable. 7) Large pieces of peeling paint was observed on the wall across from the stairway at the basement. 8) Additional refrigeration may be required, alter |
