Comments: All violations from prior inspection on 3/3/16 have been resolved except for violation 15A (see this report). Operator indicated that the carpet listed above had been approved in 2006 by unnamed individual from the NYSDOH, and that he had informed prior inspector of this fact. Operator further stated that inspector had told him that he would need to submit a letter indicating prior approval, and that inspector had said he would inform him of how ro write the letter--he said the inspector never called him to discuss the matter further. Operator was informed that this matter would be discussed with prior inspector, and that he would be contacted to indicate what he might need to do next. Food temperatures were all within proper range including sliced tomatoes at 39 degrees F. and chili at 41 degrees F. Follow up was performed in office on 10/12/16 when supervisor was informed of what operator had indicated during inspection. Supervisor indicated that operator should write a letter to the office indicating he wishes to request a waiver of the NYS Health Code

Per interview, proper cooling procedures have not been followed at this establishment. Education was provided about proper hot holding of hot dogs and proper storage of raw meats and chicken wings.
