Comments: Follow up re-inspection for continued monitoring. No live roaches noted on-site. Facility has been satisfactorily maintaining cleaning of floors, storage areas, etc. No debris noted. Reminded to continue with regularly scheduled cleaning, and should any pest activity be observed in future, to contact pest control immediately.

Comments: Re-inspection for continued monitoring of cockroach infestation. Received copy of pesticide treatment. See attached. Check of facility yielded no evidence of live roach presence. 2 dead roaches noted in glue (sticky) traps. One in rear storage, and one in paper storage on opposite end of facility. These traps were placed 9 days prior to time of inspection. Facility is continuing to maintain cleaning. Facility status is much improved. Instructed operator to continue with regular cleaning schedule, and to maintain vigilance with insect monitoring. Should roach presence return, operator is to contact pesticide company for treatment applications.

Operator has made substantial improvement in cleaning/degreasing surfaces throughout facility. **States they have also vacuumed ceiling (where fogging treatment was applied) -OK to open, per DVP. Instructed to have exterminator retreat within 1 week, and to continue monitoring. HDO will follow up with re-inspections beginning in 2 weeks. Subsequent frequency will be determined by outcome. Instructed to have a much more regimented cleaning schedule to eliminate pest attraction and harborage. Copy of exterminator report to be supplied to HDO when completed.

Comments: Re-inspection performed post pesticide treatment. Large amount of cleaning has been performed. However, some still remains. Instructed operator to clean above ceiling (vacuuming, etc), and once that is completed, he needs to clean additional areas which were overlooked; namely undersurfaces on grill/cook line. High degree of oil and food debris build up noted on undersurfaces. Also, the Grease catch trays were not cleaned. Large presence of oil, burnt food and dead roaches still noted on all trays. Sink area still needs cleaning. Instructed to also vacuum paper storage area (former walk-in cooler vault). Live roaches still noted in various locations throughout the facility. Operator to be out of town today. Will attempt to clean tomorrow. If finsihed, he will contact HDO for re-inspection Friday afternoon.

Comments: Operator states attempted (two weeks prior to re-inspection) to have pest control company come to service facility, with intention of cleaning throughout, once pesticide application completed. Operator states company was unavailable to service at time of request. Did not attempt follow up call. Operator’s wife was out of town recently with ill family member, returning evening before inspection, and therefore cleaning was not performed. Heavy cockroach presence detected, both live and dead throughout. Instructed to contact regular pest control immediately, and to contact different company, should they be unable to treat within upcoming week. Instructed to perform thorough cleaning of entire facility, focusing on walls, floors and cleaning harborage areas in walk-in cooler, and under upright refrigeration unit in main cook station area. Walls, floors, ANSUL system piping, areas in between cooking equipment, walk-in cooler shelves have been cleaned. No insects noted in facility. Also hand delivered an AT-10, but owner had already received it by mail and sent back the paperwork and fine.
