Comments: facility operating with an expired permit. permit expired on june 30, 2018. operator was notified prior to the permit expiration date and a renewal application was supplied via email. spoke to the operator via telephone this morning and it was stated that the application was mailed last week. as of 9am this morning this department has not received the application. this inspector advised the operator to have a compleated renewal application as well as the fee of $1180.00 and late fee of $80.00 on site and this inspector would pick it up. at this time the facility has an incomplete application on site. the application is missing proof of disability insurance form DB 120.1. this information cam be emailed to [email protected] or fax 914-813-5970. a bank cashiers check 9819814387 in the ammount of $1260.00 was recieved with the incomplete application. a re-inspection was also performed at this time. all violations are repeat violations re-inspection on a routine basin until facility is in compliance the basement floors, staircase, walk in box floor must be addressed by next routine inspection this matter has been referred for administrative action under case number php-7686-18-mk-vast-fse with a hearing to be held on july 24, 2018 @ 930am to be held at 25 moore avenue, mount kisco ny 10549 a representative of the corporation must attend the hearing

THESE ARE THE SAME LOCATIONS THAT ROACHES WERE OBSERVED ON MARCH 27, FEBRUARY 16, JANUARY 25, 2017 AND ALSO ON JUNE 21, 2016. PREVIOUS CASE # PHP-6379-16-MK-VAST-FSE THE CURRENT EXTERMINATION PRATICES ARE NOT ADEQUATE TO CLEAN OUT THE INFESTATION. AT THIS TIME IT IS REQUIRED THAT THE FACILITY STOP THE INFESTATION IMMEDATELY THE OPERATOR MUST PROVIDE DOCUMENTATION ON WHAT INSECTICIDES AND CONCENTRATIONS THE EXTERMINATOR IS USING INSIDE THE FACILITY.
