Comments: Case # php-7398-18-mk-rcb2-fse Hearing update re-inspection conducted at this time. Facility remains closed pending a hearing scheduled for January 25, 2018 at 9 AM at 25 Moore Avenue, Mount Kisco, NY 10549. Notice of hearing issued at time of previous inspection due to unusually large number of violations including public health hazards, rodent infestation, cockroach infestation and inadequate wiping cloth storage. Basement back exit hallway should not be used by the restaurant for any purpose, unless emergency escape route is needed. This area has rodent droppings, cockroaches and fungus growing on the walls. OPERATOR IS ADVISED AT THIS TIME THAT CORRECTION OF VIOLATIONS WILL CONSTITUTE MAJOR RENOVATION AS IT WILL REQUIRE INSTALLATION OF AND REPAIR OF SEVERAL SINKS AND RELOCATION OF OTHER SINKS WHICH WERE INSTALLED UNDER WASTE WATER LINES WITHOUT APPROVED PLANS. WILL ALSO REQUIRE REPLACEMENT OF 2 WINDOWS AND RESURFACING OF SOME AREAS OF WALLS, CEILINGS AND FLOORS INCLUDING WALK IN REFRIGERATOR AND FREEZER FLOOR. MAJOR RENOVATION NECESITATES THE SUBMISSION OF PLANS. PLEASE SUBMIT SAID PLANS TO THE BUREAU OF PUBLIC HEALTH PROTECTION AT YOUR EARLIEST POSSIBLE CONVENIENCE.
