OPERATOR IS ADVISED AT THIS TIME THAT CORRECTION OF VIOLATIONS WILL CONSTITUTE MAJOR RENOVATION AS IT WILL REQUIRE INSTALLATION OF AND REPAIR OF SEVERAL SINKS AND RELOCATION OF OTHER SINKS WHICH WERE INSTALLED UNDER WASTE WATER LINES WITHOUT APPROVED PLANS. WILL ALSO REQUIRE REPLACEMENT OF 2 WINDOWS AND RESURFACING OF SOME AREAS OF WALLS, CEILINGS AND FLOORS INCLUDING WALK IN REFRIGERATOR AND FREEZER FLOOR. MAJOR RENOVATION NECESITATES THE SUBMISSION OF PLANS. PLEASE SUBMIT SAID PLANS TO THE BUREAU OF PUBLIC HEALTH PROTECTION AT YOUR EARLIEST POSSIBLE CONVENIENCE. |
