In an attempt to ensure that the operator has been receiving the DOH Food Service Establishment annual inspection reports, the email and contact information was verified at the time of inspection by DOH. The email address for the establishment was hand written by the operator on last year's inspection report in the upper right hand portion of the inspection report. A reinspection of this establishment is required. |
