Comments: Re-inspection of facility following complaint investigation on 7/7/15. Facility was required to complete the following items by close of business on 7/9/15: 1 - Repair or replace reach-in cooling unit in kitchen. 2 - Install an acceptable refuse container in the garbage area. 3 - Contract a pest control professional regarding rodent infestation. 4 - Maintain sanitizer on premises. 5 - Conduct manual sanitization of all dishes, pots, pans, utensils, etc. The results are as follows: 1 - This item has NOT been addressed. Professional repair scheduled for later this evening, 7/10/15. Facility is to continue to use restricted menu until adequate refrigeration has been restored. 2 - This item has been adequately addressed. Dumpster has been installed. 3 - This item has been adequately addressed. Pest control company visited facility on 7/9/15. Mechanical traps were installed in facility. 4 - This item has been adequately addressed. Bleach and eco-lab brand sanitizer was present. 5 - This item has been adequately addressed. Dish washing machine was repaired during inspection and now appears to be operating according to manufacturer's instructions. Facility operator had previously agreed to correct the violations noted on this inspection report by 4/1/15 and to contact this Department. The violations were not corrected and the Department was never contacted regarding a change to the agreed compliance schedule. This Department is requiring that the facility submit proof of correction of ALL of the above noted violations by 6/20/15. Proof is to include photographs of corrections as well as any invoices from plumbers, refrigeration technician, etc. Submit proof to the Geneva District Office at [email protected].
