UPON ARRIVAL MET WITH JODY L. LEVERICH (PC FIRE INSPECTOR). SAME EXPLAINED ISSUE AND FINDINGS. OBSERVED BASEMENT FILLED WITH SEWAGE AND ODOROUS LIQUID. FACILITY HAS A DOOR IN THE FOOD SERVICE AREA WHICH LEADS TO THE BASEMENT. ODORS ARE PRESENT IN FOOD SERVICE AREA DURING INSPECTION. OPERATOR WAS REQUESTED TO CLOSE / CEASE FOOD SERVICE OPERATION. OPERATOR VOLUNTARILY CLOSED FACILITY DURING VISIT. FOOD SERVICE OPERATION IS TO REMAIN CLOSED UNTIL SANITARY WASTE LINE IS REPAIRED, BASEMENT IS PUMPED OUT AND AREA IS SANITIZED. FOOD OPERATION MUST CONTACT HD BEFORE IT CAN REOPEN. |
