Comments: CCHD has received repeated complaints for mice and cockroaches. No improvement in pest control observed after several inspections: numerous dead mice observed on sticky traps and evidence of mice eating rice and pasta in the basement dry goods, and dead cockroaches . No evidence of pest control in the basement observed. The building must be mouse- proofed, food items put in mouse- proof containers in the basement and a professional pest control company must be hired on a year- around basis. The kitchen equipment and floors must be completely cleaned, degreased and sanitized and maintained. Legal action is pending and a reinspection will be scheduled. Please provide receipts for pest control company.

Comments: All items cited on Inspections Report on March 14, 2017 have all been satisfactory corrected with exception of floor underneath fryer. Please clean this area completely and install a non-leaking fryer ASAP and continue with pest control and prevention by sealing off entry points to kitchen and removed all food sources on the floor. The rusted storage shelves in basement walk-in should also be addressed this season.

Comments: No hot hold food was ready at time of inspection: Italiian wedding and ovgo lemono soup were being reheated. Ovgo soup was at 165 degs . A reinspection will be scheduled in 2 weeks and requires a thorough general kitchen cleaning and pest control services (please have receipts) and $50 fee reinspection with check made out "Director of Finance" upon arrival. I strongly encourage you send one or more cooks to food safetty training. Next County class is on March 31 at 1-4 pm, Mayville Chautauqua Suites. Gave food safety brochures, posters and ill food worker policy. Spoke with owner on the phone.
