Comments: Discussed complaint with operator. Operator states that someone did come in to talk to him about eating at the facility and getting sick afterword. He said that he discussed with them that the facility had no other complaints. Education provided that anytime an illness complaint is received NYSDOH GFDO must be notified within 24 hours. All violations to be corrected by 4/25/17. Please send a list of each violation and the corrective actions taken by 4/25/17.
