Comments: Sanitation and food handling methods in the establishment are greatly improved. The establishment was clean and free of clutter, all foods in cooler units were properly covered. All foods found in the steam table and cooler units were maintained at proper holding temperatures. No evidence of roaches was observed during the inspection. Please be aware that the Cattaraugus County Board of Health voted to deny "El Mariachi" renewal of its Food Service Establishment permit at the end of September unless someone on staff has successfully completed a CFP/ANSI accredited food manager certification program and provides proof of completion to the Health Department. As of this inspection, this requirement has not been met.

Comments: Pest control receipts provided by operator during the inspection. One pest control report dated Dec 12, 2016, indicated that the establishment was treated as part of a "monthly preventative" visit and that the roach activity in the establishment was "greatly reduced". Following a customer complaint over the weekend, a second visit by the pest control company was made earlier today at the request of the restaurant. This report indicated that no roach activity was observed by the pest control operator. However, several dead roaches were observed during my inspection of the facility conducted on the same day. Restaurant operator indicated that they believed they had eliminated the roach infestation earlier in the year and had not experienced any insect issues until just this last weekend. He plans to resume having weekly pest control treatments until the problem is resolved. Copies of pest control reports must be provided to this department for documentation within three business days following the treatment.
